How to be a patient: Crescent Valley Medical Center accepts new patients. It is appreciated if you call in advance for an appointment. The Medical Center welcomes most private insurance, Medicaid, Medicare, Women’s Health Connection and Nevada Check-Up. A sliding fee schedule for low-income patients who do not have insurance is available. Identification and income information is needed to enroll in the sliding fee program.
Appointments are strongly recommended; some sites do see walk-ins for emergency needs. Call the medical center to check on walk-in availability.
Prescription Refills: Ask your pharmacist to fax your request for prescription refills to the doctor; this will speed up your refill process.
Additional Information: Crescent Valley Medical Center, located in Eureka County, opened its doors to serve the community with quality health care in 1999. Laurel Kleinman, APRN, is a Nurse Practitioner. She received her Master of Science in Nursing, emphasis in Family Nurse Practitioner from Graceland University. She began working in the healthcare industry in 1981 as a Medical Assistant and aspired to be a Nurse Practitioner. Her dream job would be to see rural patients who could not afford medical care in their home and receive payments in forms of apple pie, eggs, chicken, tomatoes, etc.
Jason Rusk, PA-C, says he appreciates the vision and leadership within Nevada Health Centers and enjoys the cohesive nature it is striving for within each of the communities it serves. Outside of work he enjoys running, cycling, photography, camping, hiking and fishing.
