How to be a patient: Carlin Community Health Center accepts new patients. It is appreciated if you call in advance for an appointment. The Medical Center welcomes most private insurance, Medicaid, Medicare, Women’s Health Connection and Nevada Check-Up. A sliding fee schedule for low-income patients who do not have insurance is available. Identification and income information is needed to enroll in the sliding fee program.
Appointments are strongly recommended; some sites do see walk-ins for emergency needs. Call the medical center to check on walk-in availability.
Prescription Refills: Ask your pharmacist to fax your request for prescription refills to the doctor; this will speed up your refill process.
Additional Information: Carlin Community Health Center began providing medical services in Carlin in 1999. The population of Carlin is 2,018 but patients come from all over Elko County seeking medical care. Carlin is the oldest community in Elko and there are several gold mines still operating in the area. For more information about Carlin, click here.
Emilse Peraza, PA-C, is a board certified Physician Assistant and has been with Nevada Health Centers since 2012. He received a Bachelor of Science in Agribusiness Management in Venezuela; a Radiology Science Studies degree from Weber State University in Ogden, Utah; and his Master of Physician Assistant Studies from the University of Utah in Salt Lake City, Utah. Emilse enjoys practicing in this rural area and is fluent in spanish. His hobbies include amateur radio, music and traveling.
