How to be a patient: Wendover Community Heath Center accepts new patients. It is appreciated if you call in advance for an appointment. The Medical Center welcomes most private insurance, Medicaid, Medicare, Women’s Health Connection and Nevada Check-Up. A sliding fee schedule for low-income patients who do not have insurance is available. Identification and income information is needed to enroll in the sliding fee program.
Appointments are strongly recommended; some sites do see walk-ins for emergency needs. Call the medical center to check on walk-in availability.
Prescription Refills: Ask your pharmacist to fax your request for prescription refills to the doctor; this will speed up your refill process.
Additional Information: Wendover Community Health Center began providing medical services to the communities of Wendover, Utah and West Wendover, Nevada in 1999. Emilse Peraza, PA-C, is a board certified Physician Assistant and has been with Nevada Health Centers since 2012. He received a Bachelor of Science in Agribusiness Management in Venezuela; a Radiology Science Studies degree from Weber State University in Ogden, Utah; and his Master of Physician Assistant Studies from the University of Utah in Salt Lake City, Utah. Emilse enjoys practicing in this rural area and is fluent in spanish. His hobbies include amateur radio, music and traveling.
Lori Hamilton, PA-C is a nationally certified physician assistant who graduated from the MEDEX Physician Assistant program through the University of Washington in August 2013. She also has a Bachelor of Arts degree from Midwestern State University, 1993.
For the past two years, she has worked in rural family medicine and urgent care in Anchorage, Alaska. She has special interest in Women’s Health and Pediatrics, as well as Family Medicine.
She is married, and has two grown sons. When not at work, you can find her hiking with her dogs, or curled up reading a good book. She loves to travel, explore new places and meet new people.
Her passion for patient care stems from her childhood desire to always help others, and care for those in need. She believes in providing compassionate, evidence-based medicine to her patients, and always puts her patients first. She strives for provider/patient teamwork and believes that patient education is vital for positive, patient care.
Lori is an active member of the American Academy of Physician Assistants.
