How to be a patient: Elko Family Medical and Dental Center accepts new patients. It is appreciated if you call in advance for an appointment. The Medical Center welcomes most private insurance, Medicaid, Medicare, Women’s Health Connection and Nevada Check-Up. A sliding fee schedule for low-income patients who do not have insurance is available. Identification and income information is needed to enroll in the sliding fee program.
Appointments are strongly recommended; some sites do see walk-ins for emergency needs. Call the medical center to check on walk-in availability.
Prescription Refills: Ask your pharmacist to fax your request for prescription refills to the doctor; this will speed up your refill process.

Travis Wies, MSN, APRN, FNP-C, worked as a Registered Nurse at Primary Children’s Medical Center in Salt Lake City, Utah. During that time he went to Westminster College to become a Family Nurse Practitioner. After becoming certified as a Nurse Practitioner he worked in Urgent Care, Family Practice and Occupational Medicine clinics in Blackfoot, Idaho. Currently he works as a Family Nurse Practitioner in the Nevada Health Centers clinic in Elko, Nevada providing Family Medicine to the residents of Elko and the surrounding areas. He is nationally certified as a Family Nurse Practitioner, American Academy of Nurse Practitioners and also a member of Sigma Theta Tau.
He is focused on providing high quality care by treating the whole individual including physical, mental, social, spiritual and emotional health. He works with his patients to develop an individualized plan of care to help them achieve their highest level of health and wellness. He believes that by including patients in the development of their care they become empowered and take ownership of their own health. He and his team are devoted to providing high quality care and improving their quality of life. He is especially interested in the care of diabetes, pediatrics and acute care.
Travis is married with four children. He enjoys spending time with his family traveling, soccer, swimming, music and outdoor activities such as camping and hiking.
John Tkach, MD, received his medical degree from University of Rome Medical School. A practicing physician for nearly 40 years, he has particular interest in Cardiology (including treadmill testing, ECG and Holter monitoring), Diabetes Management, DOT?CDL Certification Exams, Wellness Exams, and Sports Physical Exams.
Taylor Crowther, PA-C, is a board certified Physician Assistant. He received his Master of Medical Science in the Physician Assistant Program at Mercer University in Atlanta, Georgia. He has a background in exercise, nutrition, and weight loss counseling.
WOMEN’S HEALTH SERVICES
Peter Beale, MD, is a board certified Obstetrician and Gynecologist that received his medical degree from the University of Pittsburgh. He completed his residency at Kaiser Permanente Hospitals in Los Angeles. In his free time he enjoys snowshoeing, hiking and camping.
Tami Baumann, APRN, WHNP, CNM, MS, is adjunct Faculty at the University of Nevada, Reno Orvis School of Nursing where is a clinical preceptor for midwives and family nurse practitioner students for Orvis School of Nursing and Frontier.
In addition to earning an Associate of Applied Science in Nursing, she completed the Harbor UCLA Women’s Health Nurse Practitioner certificate program, earned a Bachelor of Science in Health Arts from the University of St. Francis, earned a Master of Science in Midwifery at Philadelphia University and will receive her Post-Masters in Nursing Education from the Orvis School of Nursing Spring 2016.
Jacob K. Butler, DMD, earned his Bachelor of Arts degree from Southern Utah University in Cedar City, Utah and his Doctor of Dental Medicine from the University of Puerto Rico. After dental school Dr. Butler spent time teaching as an Adjunct Faculty for the Arizona School of Dentistry & Oral Health. He has also worked in dentistry in rural Arizona and Washington. He loves small towns, and is happy to have joined the Elko Family Medical & Dental Center in 2015. He is a member of the American Dental Association, American Association of Endodontists, American Dental Education Association and the Academy of LDS Dentists. He is married and has four children. In his free time he enjoys golfing, fishing, hiking and spending time with his family.
Daniel Egbert, DMD, graduated with Honor from the Oregon Health and Science University in Portland, Oregon and was one of three individuals to graduate Magna Cum Laude with a Bachelor of Science in Human Biology from Brigham Young University in Rexburg, Idaho. He is fluent in Spanish, has advanced dental procedure experience and was active in leadership positions coordinating annual health events in the community.
Bryce S. Putnam, DMD, received his Doctorate in Dental Medicine from University of Nevada Las Vegas, School of Dental Medicine.
Brooke Smith, BS, RDH, is a state public health endorsed dental hygienist for Nevada Health Centers. She graduated with her A.S. in Dental Hygiene from Carrington College, Boise in 2014, after receiving a B.S. in Biology from the University of Utah in 2012. In her spare time she enjoys running, traveling, being outdoors, and spending time with her husband, family and friends.
